"I have an off-the-shelf web app built in PHP & using a MySQL db that allows subscribers to upload their digital documents

I now need to a add a form to the app that allows the capture of customer data from a set of predefined questions once they are logged in and which they can amend or add to at any time.

The form, which I will create in PHP, will comprise of approx 8 sections with a total of 35 questions and will be a single page form.

To make the form more user friendly I would like some features added which includes

• Each question to include the option to be disregarded by the user. Disregarded questions should then be removed from the main form and appear at the bottom of the page in a disregarded section along with a button to allow question to be reinstated back into the form if required
• Count of questions answered for be displayed for each section and whole form
• Every section which has been fully or partly completed needs to generate a PDF version of any inputted data that includes Section title, questions and related inputted data . Form should contain a link in each section where PDF can be downloaded. If no data is entered into a section no link should show. Disregarded questions should not appear in the PDF's
• A PDF that includes all inputted data should also be generated along with an associated downloadable link
• A few questions will require the ability of the user to add another line of data dynamically so an "Add new" input Line functionality is required eg: Questions asks name of bank and account type and customers has more than one bank account or account type
• A few questions will require dynamic branching based on the initial question
• All input fields should auto save following input with "Saving progress ..." and "Saved" notifications shown to user.


Note: I want the form to remain written in PHP so as to make it easier to make changes and amendments by non-developers.
