"I need several new Box Forms set up inside our Box environment such as conflict of interest declaration, once submitted, the forms will be stored in the correct HR folder with the right metadata.

Here’s what I’m after:

• Build clean, user-friendly Box Forms (using the native Forms builder or Box UI Elements + Metadata API) configured for public access so anyone with the link can submit, yet no one can browse existing files.
• Capture the following fields: First Name, Last Name, Email (by auto-lookup from AD).
• Map each field to an existing or newly created metadata template so the information is immediately searchable once it lands in Box.
• Route every submission to the folder structure I’ll specify and confirm end-to-end functionality with test entries.
• Hand over a short set of instructions and or a quick Zoom video—so my team can replicate or tweak the forms later.

Acceptance criteria
• Form loads with organisation Box login.
• Submissions appear in the target folder with all metadata correctly attached.
• No unintended permissions or content visibility are exposed.
• Clear, reusable documentation is delivered.
