"I need a secure, modern-looking login page built with PHP and MySQL. The page should authenticate users against a MySQL table using prepared statements and password_hash / password_verify, then start a session on success. Visually, it has to follow a clean, contemporary style—feel free to lean on Bootstrap 5 or an equivalent framework as long as the final HTML/CSS stays tidy and easy to tweak.

Functional requirements:
• Password reset: generate a secure token, email it (or store it for manual testing), and let the user set a new password through a simple form.
• Basic error handling: show clear, inline messages whenever credentials are missing or incorrect, and log any repeated failed attempts server-side for future rate-limiting.
• User registration is not needed right now, but I’d like the code structured so the feature can be slotted in later without a full rewrite.

Deliverables:
• SQL script that creates the users table (id, email, hashed_password, reset_token, reset_expiry, etc.).
• PHP files for login, logout, and password-reset flows, plus a minimal config file.
• One responsive HTML/CSS template implementing the modern design.
• A concise README explaining setup steps and where to adjust SMTP or other environment variables.
