"I need a simple Google Form that functions as a feedback tool. The form must collect a visitor’s name and email, phone number, and an open-ended message or comments field, then let the user finish with a single Submit button. All entries should flow straight into the linked Google Sheet so I can review and filter responses later.

Please add my logo and brand colours (assets supplied) and switch on reCAPTCHA plus email notifications for every new submission. When the form is live, send me the shareable link and the iframe embed code so I can drop it into my site immediately.
