"I need a small Power App that lets users run a customer survey aimed at assessing service quality. The app will contain three identical sections; each section holds three questions. Each question is answered with a single-choice radio button scale from 1 to 5.

Every time a respondent submits, the app must append a row to an external CSV file with the following columns in this exact order:
• Date (submission timestamp)
• Name (typed by the respondent)
• Section number (1-3)
• Question (the text of the question)
• Response (1-5)

After writing to the CSV, the same data should flow into Power BI and populate a simple table visual so I can track responses in real time. A refresh schedule or direct connection that keeps the report current is fine—whichever is simplest to maintain.

Acceptance criteria:
1. A packaged, share-ready Power App file (.msapp) with the three-section, three-question layout and 1-5 radio scale.
2. Automated export of each submission to a CSV stored in OneDrive or SharePoint.
3. A Power BI file (.pbix) or workspace report that reads the CSV and displays the data table instantly on refresh.
4. Clear setup notes so I can redeploy or move the files if needed." (client-provided description)
