"I need a web application that captures, stores and reports on the annual medical examinations carried out for factory employees and school students. The portal will operate with three distinct roles:

• Admin – full user management, the ability to generate high-level reports and run basic data-analytics dashboards (filters, charts, trends).
• Doctor – secure entry of examination results, instant access to each patient’s historical records, and one-click generation of individual medical reports in PDF.
• Factory/School Representative – upload or type employee / student lists, schedule examination slots, and download consolidated health summaries for their organisation.

A clean, responsive interface (desktop and tablet) and a relational database backend are both essential. Role-based permissions should ensure each user only sees what they need, while all data transfers must be encrypted (HTTPS). Standard exports to Excel/CSV and printable PDFs are required so stakeholders can archive results offline.

Deliverables:
1. Source-controlled codebase (frontend, backend, database schema)
2. Deployed test instance with sample data
3. Brief setup & user guide
