"Hi,

I am a medical device distributor for lymphedema pumps. I currently only distribute lymphedema pumps and as I grow my business I would like to expand into compression wraps.

I am interested in setting up a website that will not only have content for the lymphedema pumps I am selling, but also be portal to place orders for:

1. My 1099 reps
2. Doctors offices that I have collaborated with and shown how to order pumps on my website.
3. New patients that want to make an place an order OR make an inquiry.

A contact form will have to be made available that let's the user identify as an HCP, or a patient that is making and inquiry, or a patient that would like to place an order.

It would be nice to keep track of all of these inquiries on a separate spreadsheet that automatically populates that is separate from the website itself.

I would like to also have a way for my customers (Doctors, HCPs, etc.) to start and order through an intake form. Tis can be done with a fillable form - wether it be docusign or Adobe...we can figure that out later.

I would like to make an intake process for my 1099 distributors too.

I would also like a blog page - for SEO purposes. My guess is I would have 3-4 pages. A content page for my devices on the home page....about us...how to order with detailed instructions .and then a blog.

Based upon this, and there might be some other modern items included I wanted to see if you were able to take on this project and what your fee would be.
