"Convert Existing FormDesigner Layout to Fluent Forms (Unlimited Domain License Required)

Project Description:

We’re seeking an experienced Fluent Forms developer (with an Unlimited Domain License) to recreate an existing form currently built in FormDesigner.

This form — titled Business Evaluator & Accelerator Marketplace — needs to be graphically improved and rebuilt using the advanced look, features, and functionality of Fluent Forms.

The form uses a matrix-style layout with:

MSP Revenue Levels across the top

Operational Categories (8 key areas of business focus) down the side

It includes multiple-choice fields, conditional logic, and structured data collection — similar to a Likert / Matrix Grid used for benchmarking and scoring within the MSP Business Evaluator & Accelerator framework.

Project Scope:
Phase 1 (Initial Project)

Convert the FormDesigner form to Fluent Forms.

Recreate all field groups, conditional logic, and table-style layouts accurately.

Ensure Fluent Forms supports Excel/CSV export with clearly labeled rows (categories) and columns (MSP peer group revenue levels).

Match and enhance the design for improved usability and professional presentation.

Fully utilize Fluent Forms’ modern UI features, including multi-column layouts, inline validation, and progress indicators.

Phase 2 (Additional Projects)

Two additional forms will be provided for conversion once Phase 1 is completed to satisfaction and on acceptable terms.

Requirements:

Must have an active Fluent Forms Unlimited Domain License.

Proven experience creating complex Fluent Forms (please include portfolio or live samples).

Please provide samples of Fluent Forms you have done in the past, including at least one form that uses a matrix or table-style layout.

Strong understanding of:

Matrix / Table / Likert field configuration

Conditional logic and scoring

Multi-step and data-driven form design

Export formatting and reporting compatibility

Fluent Forms must generate a unique edit link that allows:

Users to return and edit their previous submissions.

Admins to securely access and edit submission data.

Ensure data persistence, a clean modern UI, and proper security for edit-link functionality.

Clear communication and reliable delivery are essential.

Deliverables:

A fully functional Fluent Form that replicates and improves upon the existing FormDesigner version.

Export-ready configuration aligned with benchmark data collection and reporting.

Documentation outlining setup, logic, scoring, and edit-link configuration.

Bonus:

Experience developing evaluation or benchmarking tools for Managed Service Providers (MSPs) or Channel Vendors is a strong advantage.
