"I want a clean, eye-catching “Partner With Us” button added to my website. When a visitor clicks it, a contact form should slide in or pop up—no redirects—so the user stays on the same page. The form needs four fields exactly: Name, Email, Phone Number, and Message.

Key points
• Platform: standard website (HTML/CSS/JS or a lightweight plugin for WordPress is fine).
• Action: clicking the button opens the contact form; it does NOT dial a number.
• My phone number and my phone-mail address must be displayed near the button or at the top of the form so shippers can reach me directly if they prefer.
