"I need a single data-entry form that feeds straight into an existing Google Sheet. The form has to accept text, numbers, and dates, be fully responsive so it feels native on both desktop browsers and mobile devices, and—once someone presses “Submit”—show the visitor a neat summary of exactly what they just entered rather than a generic thank-you page.

You are free to build it in vanilla HTML/CSS/JS, Google Apps Script, or another lightweight approach that keeps deployment simple (I should be able to publish it as a web app or embed it in an HTML page with minimal setup). What matters is:

• Every field maps cleanly to a column in my sheet.
• It works and looks good on phones and larger screens without extra zooming.
• The post-submit screen displays the submitted data in an easy-to-read format.
