"I want a web-based application form that routes each submission to a 2 staged approval process based on submission value threshold. The solution can be built in the tool you’re most comfortable with—Google Workspace add-ons, Microsoft Power Automate, Jotform, or a lightweight custom page—as long as it’s easy for me to update later. I have created a form with the fields and details required in Microsoft Forms - this can be used as a template or form part of the solution.

Must-have functions
• File upload so applicants can attach supporting documents
• Dynamic fields that reveal or hide questions based on previous answers
• Email notifications to the applicant and to the manager when the form is submitted, approved, or rejected
• Form details, status, with submission and approval dates to be captured on a register (spreadsheet or SharePoint list)

Nice-to-have
A simple status view where I can see pending and completed requests.

Deliverable
A working form with the approval flow configured, plus short hand-over notes so I can change field labels and email text myself.
