"I need a web-based dashboard that lets my clinic manage patient records smoothly while giving patients a secure portal to enter their own medical history. The core of the build is records management—no appointment calendar or billing module for now—so every screen, workflow, and database decision should revolve around capturing, storing, and retrieving a detailed medical history.

Patients will log in through a dedicated portal, fill out their history before a visit, and see only their own data. On the clinic side, staff should be able to review, edit, and add notes from any device. A clean UI, quick search, role-based permissions, and strong security (HIPAA-level encryption, secure authentication) are essential. I’m open to your preferred stack as long as it scales and stays maintainable; popular choices like React, Vue, Laravel, Django, or Node are all fine if you can defend them.

Deliverables should arrive in workable chunks so we can test early:

• Functional patient portal with self-entry forms and validation
• Admin dashboard for staff to view, edit, and search medical histories
