"I need a straightforward mobile app that runs on both iOS and Android so our drivers can complete their daily car / truck inspection right from their phones.

The workflow is simple: the driver opens the app, fills out a form made up entirely of text-entry fields, attaches photos or other files when necessary, and taps submit. No dropdowns are needed. As soon as they send the form, the data and any files should be pushed to a secure back-end endpoint or cloud storage that my office team can review and archive.

Because the app will be used on the road, it has to launch quickly, work offline when reception drops, and queue submissions until the device reconnects. A clean, minimal UI is fine; reliability and speed are what matter most.

• Full, well-commented source code
• A short README explaining how to adjust form fields, branding, and the submission endpoint
