"I need an experienced developer who can immediately restore the submission flow of the web application on federaldirectfunding.com. Right now, form data is no longer reaching either my admin portal or my inbox, and I need that fixed today.

Scope of work
• Trace whatever break is preventing data from being written to [login to view URL]
• Make sure the same submission is also sent to my designated email address
• Verify both paths with a live form test so I can see the record inside the admin dashboard and receive the matching email notification
• Keep all existing functionality and styling intact—this is strictly a reconnection job, not a redesign

Tech environment
The site is built with standard PHP/MySQL on a cPanel host. You will have FTP and database access the moment we start.

Acceptance criteria
1. Submissions appear in the admin panel without errors
2. Identical submission arrives in my email inbox
3. You outline the change in a short note so I can replicate it later if needed
