"I already designed my one- and two-page consultation and after-care patient forms in Canva (Pro account). Now I need them turned into fully fillable documents that patients can complete on my iPad during their visit and electronically sign.

Here’s what I’m aiming for:
• The form stays visually identical to my current Canva design, but every text field, checkbox, date picker and signature line must be interactive.
• When a patient finishes, the form should save automatically as a PDF and name the file with the patient’s name so it sorts alphabetically in cloud storage—Dropbox, Google Drive or iCloud, whichever you recommend and can configure.
• The workflow has to be seamless for non-technical staff: open form on iPad, hand device to patient, hit “save” and the file lands in the correct folder without any extra steps.

Please set up whatever connectors or automation are required (Canva export, Acrobat, Jotform, Zapier, Shortcuts, etc.) and walk me through the process so I can replicate it for future forms. A short screen-recorded demo of the finished workflow will serve as the final acceptance criterion." (client-provided description)
