"I’m upgrading my site and now need a straightforward username-and-password login area that I can drop straight into an existing PHP/HTML stack. The form itself must be clean, responsive and accessible, but nothing fancy beyond the essentials—speed and security matter more than visual effects.

Three features are non-negotiable:
• “Remember me” checkbox that keeps the session alive with a secure cookie.
• “Forgot password?” link that points to a reset endpoint I’ll supply.
• Google reCAPTCHA (v2 or Invisible) to block bots.

I’ll wire it to my own user table, so your job is to deliver:
1. The front-end markup (HTML5 + CSS3) with graceful mobile fallback.
2. Lightweight JavaScript to handle validation and captcha callbacks.
3. A sample PHP endpoint that receives the POST, hashes the password (bcrypt), sets the session and honours the remember-me token.
4. A short README explaining where to plug in my DB credentials and reset URL.

No frameworks are required, but if you prefer a micro-framework such as Slim or a single JS helper library, that’s fine—just keep the footprint minimal and document any dependencies.

Once I can drop your files onto my server, change the DB creds and see successful login, the job is done." (client-provided description)
