"Medical office dashboard and task tracker.
Up to 10 departments each with custom names
Custom KPIs for each department usually up to 15 KPIS
KPIs will also have individual metrics that feed the KPIs
Data entry preferred both phone and web app but web takes precedence.
Data entry combined manual entry and Excel/csv extraction
Each kpi and metric is line or bar graphed with flexible date extraction points
Each kpi and metric will have targets and some will have optional baseline and incentive targets.
Individual and department summaries will have report cards that are derived from weighted metrics
Data entry will come from various departments but can be applied to other departments (like attendance entry from admin but applies to nursing department)


The app will also have a task tracker with responsible parties, progress tracker, and involved parties including a flag for executive input required when additional direction is needed. Each task will have red yellow green status or progress updates on a slider for proper progress over time.
The task reports will then generate a weekly summary for each department to review and update which then will generate a single report to the CEO comprising of 3 sections.
1) projects needing executive input
2) dashboard of KPIs from each department
3) individual tasks per department and notes on progress.

This can become an ongoing project but I first want an estimate on the dashboard area and task tracker with ongoing updates once the first version is deployed. This will be an ongoing project so I need an initial estimate for v1.0 to deploy with additional cost per hours used or revisions needed." (client-provided description)
