"Need a mobile-friendly web app (not a native app) that allows our field team to complete a daily checklist and submit it by email.

Key Features:
Works on all modern smartphones (browser-based).
Around 25 checklist items, each with two options:
Complete, no issues
Not complete (with short text explanation).
At least two versions of the checklist.
Built so new items can be added later without a full rebuild.
Generates a clean, formatted email report including:
Checklist results
Job details (event, date, location, submitter info)
Fixed recipients (always included) + user-chosen recipients.
Must run on our DigitalOcean VPS (Linux environment).
Email sending must use Mailgun (account already exists).
Deliver full source code + setup/maintenance documentation.
We are looking for someone who communicates clearly, is receptive to feedback, and provides a clean handoff at the end.

Deliverables:
Functional web app deployed on our DigitalOcean VPS.
Mailgun configured for email delivery.
Source code and setup documentation.
Instructions for adding checklist items in the future.
