"Bestchemist – Workflow Summary

Step 1 – Booking Page

Show 3 product boxes ($9.99): Sick Leave (Work), Sick Leave (University), Carer’s Leave.

On click → go to Personal Details Form.

Product type saved as hidden value.


Step 2 – Personal Details Form + OTP

Fields: Full Name, DOB (must be 16+), Address (Google Autocomplete, no PO Boxes), Phone (AU format), Email, Certificate Day (Today/Tomorrow only), Notes.

After entering Phone → OTP sent immediately via SMS (or fallback Email).

User must verify OTP before proceeding.

Validation errors for <16, PO Box, missing fields.

Once OTP verified → go to Conditional Questions.


Step 3 – Conditional Questions

Sick Leave: Must confirm mild illness, not severe symptoms, no duplicate doctor cert, understands questions, checkbox confirmation.

Carer’s Leave: Must confirm person is unwell/injured, not severe symptoms, understands questions, checkbox confirmation.

If invalid → error/warning messages.


Step 4 – Payment

Redirect to Stripe Checkout link.

Only after successful payment → submission saved and Thank You page shown.


Step 5 – Thank You Page

Simple message confirming submission and review by health professional.


Step 6 – Emails

Admin Email: All patient details, Q&A responses, OTP verified, Payment successful.

Patient Email: Confirmation of submission, certificate will follow shortly.


Step 7 – Certificate Templates

Sick Leave & Carer’s Leave templates (with logo, date, patient details, pharmacist signature).

No contact details in footer.

Sent manually after review.
